Über Prof. Dr. med. Alexander Albert

Seit dem Herbst 2019 hat das Herzklinikum in Dortmund einen neuen Leiter: Mit Prof. Dr. Alexander Albert konnte das Klinikum Dortmund einen renommierten Herzchirurgen verpflichten, der über eine außergewöhnliche Expertise verfügt. Mehr als zwanzig Jahre Berufserfahrung in leitender Funktion kann der Facharzt vorweisen, der gleich in mehreren kardiologischen Bereichen als ausgewiesener Spezialist gilt.

Das Klinikum Dortmund, eines der größten Krankenhäuser in Deutschland, hat mit der Besetzung der Chefarztposition der Herzchirurgie durch Prof. Dr. med. Alexander Albert einen starken Akzent auf der Herzmedizin gesetzt. Der neue Klinikdirektor im Herzzentrum war zuvor zehn Jahre lang Stellvertretender Direktor der Herzchirurgie am Universitätsklinikum Düsseldorf – und hat diese Klinik zu einem der bedeutendsten Herz- und Transplantationszentrum mit aufgebaut. Zuvor war er zehn Jahre Oberarzt im Herzzentrum Lahr/Baden gewesen, einer renommierten Privatklinik in Süddeutschland, darüber hinaus erwarb er sich in besondere Spezialisierungen im Universitätsklinikum Jena sowie im europäischen Ausland: im Kantponsspital Basel in der Schweiz und dem Universitätsklinikum Leuven (Belgien).

Mit dieser langjährigen Erfahrung und seinem neu aufgestellten, international besetzten Team entwickelt er die Herzchirurgie des Klinikum Dortmund nun zu einem überregional bedeutenden Zentrum. Die Tatsache, dass der neue Klinikdirektor für Patienten aus dem Umland eine im bundesweiten Vergleich außergewöhnliche Breite an chirurgischen Behandlungsmöglichkeiten bietet, macht die Kooperation mit dem Klinikum Dortmund äußerst interessant.

… auf die Bedürfnisse des Patienten eingehen

Bei aller Erfahrung und allem internationalen Renommee: Im Vordergrund steht für Prof. Dr. med. Alexander Albert immer der Patient mit seinen ureigenen Bedürfnissen und Wünschen. Prof. Albert legt größten Wert darauf, dass diagnostische und therapeutische Vorgehensweisen im interdisziplinären Team besprochen werden. Das schließt nicht nur Kardiologen und niedergelassene Ärzte ein, das betrifft auch die Patienten selbst – und deren Familie. Ob der Klinikdirektor selbst oder einer der Oberärzte bei den „Heart-Team-Besprechungen“ dabei ist, spielt keine große Rolle. Wichtig ist schließlich, dass am Ende genau das Verfahren gefunden wird, das möglichst schonend, aber auch mit Blick auf die langfristige Prognose und für die Lebensqualität der Patienten das Optimum darstellt.

Auch komplexe und seltene Eingriffe mit hoher Qualität

Das neue Team unter seiner Leitung ist so aufgestellt, dass jeder Herzchirurg ein Spezialgebiet hat, auf dem er besonders viel Erfahrung und Fachwissen besitzt; nur so können auch seltenere oder sehr komplexe Eingriffe in hoher Qualität durchgeführt werden. So werden in Dortmund selbst äußerst schwierige und seltene Operationen mit einer einzigartigen Mischung aus konzentriertem Forschungsdrang und bewährter Routine absolviert.

Zum Leistungsspektrum des Teams um Prof. Albert zählen unter anderem die Entfernung infizierter Schrittmachersonden, die minimal-invasive Bypass- und Herzklappentherapie, innovative Aortenchirurgie, komplexe Endokarditis-Operationen – und die Ross-Operation, bei der eine defekte Aortenklappe durch die körpereigene Lungenschlagaderklappe ersetzt wird. Auch der totale Herzersatz mittels eines Kunstherzens wird in Dortmund durchgeführt.

Entscheidend für den Klinikdirektor ist, dass die gesamten Möglichkeiten der Herzchirurgie vorgehalten werden, um den Patienten eine personalisierte Therapie anbieten zu können. Der Nachweis der Expertise in den einzelnen Therapie-Bereichen ergibt sich dabei aus den Lebensläufen der Ärzte und aus den umfangreichen OP-Katalogen. Das gilt nicht nur für Prof. Albert, sondern auch für seine Oberärzte. Auch einschlägige Publikationen zu den jeweiligen kardiologischen Themen sprechen für das Team.

International führender Spezialist für moderne Bypass Chirurgie

Prof. Albert gilt als einer der international renommiertesten Spezialisten für die Off-Pump-Chirurgie und die minimal-invasive Bypass-Chirurgie. 2004 absolvierte er einen einjährigen Aufenthalt bei Prof. Sergeant in Belgien, damals Europas führender Spezialist für Bypass-Operationen ohne Herz-Lungen-Maschine. Eben jenes Verfahren führte Prof. Albert danach in der Privatklinik Lahr ein – und machte die Klinik – gemeinsam mit dem damaligen Klinikdirektor Prof. Dr.med. Jürgen Ennker, in einzigartiger Weise zu einem führenden Zentrum weltweit auf diesem Gebiet. Prof. Albert entwickelte nicht nur die Bypass-Chirurgie weiter, er leitet nun auch das europäische Trainingszentrum für minimal-invasive Bypass Chirurgie der Firma Medtronic in Dortmund.

Prof. Dr. med. Alexander Albert ist einer der wenigen Chirurgen weltweit, die Mehrfach-Bypass-Operation über minimal-invasive Zugänge in täglicher Routine durchführen – und zwar ohne Sternotomie. Entscheidend ist für ihn aber auch hier, zusammen mit dem Patienten und den behandelnden Ärzten ein personalisiertes Therapiekonzept zu entwerfen. So ist beispielsweise aufgrund der individuellen Gegebenheiten für den einen Patienten häufig ein minimal-invasives Vorgehen gut möglich, für den anderen vielleicht eine Kombination aus Stent und Bypass besser, also ein sogenannter Hybrid-Eingriff – während für andere Patienten wiederum das klassische Vorgehen über eine Sternotomie sinnvoller ist. Dann operiert Prof. Albert allerdings auch ohne Herz-Lungen-Maschine, denn das bleibt bei ihm seit mehr als fünfzehn Jahren Standard.

Biologische, rekonstruktive Lösungen – wenn immer möglich

Zur Philosophie von Prof. Albert gehört es, wenn sinnvoll, die natürlichen, biologischen Strukturen des Herzen zu erhalten. Noch sind die derzeitig verfügbaren Prothesen nicht ideal und auf lange Sicht mit Nebenwirkungen verbunden, sie sind nach wie vor dem natürlichen Gewebe unterlegen. So werden Herzklappen in Dortmund vorzugsweise repariert und erst dann durch Prothesen ersetzt, wenn gar keine andere Möglichkeit besteht. Bei den Mitral- und Trikuspidalklappen-Erkrankungen gelingt dies in über neunzig Prozent der Fälle. Bei der Aorteninsuffizienz durch eine Dilatation des Anulus wird die Herzklappe durch eine Wiederaufhängung in eine normierte Prothese routinemäßig repariert.

Auch in anderen Bereichen, wie etwa der fortgeschrittenen Herzinsuffizienz, wird erst alles versucht, um eine schonende, möglichst nebenwirkungsarme Therapie durchzuführen, bevor maschinelle Unterstützungssysteme zum Einsatz kommen. So wurde kürzlich als einer der ersten Eingriffe weltweit in Dortmund eine minimal-invasive Ventrikel-Rekonstruktion durchgeführt, wobei das Revivent-System der Firma Bioventrix zum Einsatz kam.

Minimal-invasiv – für eine zügigere Genesung des Patienten

Prof. Albert gilt als ausgewiesener Spezialist für minimal-invasive Herzchirurgie. Die Stärke eines herzchirurgischen Eingriffs liegt darin, dass Voraussetzungen für eine Wiederherstellung einer optimalen Herzfunktion und Beschwerdefreiheit des Patienten über Jahrzehnte geschaffen werden. Diese Chance sollte durch die Art des chirurgischen Zugangs nicht riskiert werden. Andererseits ist die Wahrung der Integrität des Körpers für die Patienten ein wichtiges Thema.

Bei der Bypass-Chirurgie ist eine sorgsame individuelle Abwägung vor der Operation für oder wider ein minimal-invasives Vorgehen ganz wichtig. Entscheidend ist dabei, dass Prof. Albert durch seine besondere Expertise in minimal-invasiver Bypass-Chirurgie diesen Eingriff in hoher Qualität anbieten kann. Dagegen führt Prof. Albert Operationen wie die Mitralklappenrekonstruktion standardmäßig seit über zehn Jahren minimal-invasiv durch.

Auch der Ersatz der Aortenklappe erfolgt entweder als TAVI oder chirurgisch minimal-invasiv über eine parasternale Mini-Thorakotomie: Das Team um Prof. Albert in Düsseldorf war das erste weltweit, das eine minimal-invasive Implantation von Herzunterstützungssystemen ohne Verwendung der Herz-Lungen-Maschine durchgeführt hatte.

Für Notfälle: das „Mobile ECMO-Team Dortmund“

Bei so viel Einsatz für die Patienten versteht es sich von selbst, dass Prof. Albert auch die unkomplizierte und sofortige Versorgung von Notfällen am Herzen liegt. Aus diesem Grund verfügt das Klinikum über eine 24-Stunden-Hotline, unter der in Dortmund in dringenden Fällen immer ein Arzt zu erreichen ist.

Dieses Jahr gründete Prof. Albert zusammen mit dem Rettungsdienst der Stadt Dortmund das „Mobile ECMO-Team Dortmund“, nachdem er bereits in Düsseldorf vor zehn Jahren das „Düsseldorf Survival Network“ mit aufgebaut hatte. Dieses ECLS-Netzwerk war sehr innovativ und hat international viel Anerkennung gefunden.
Das Konzept besteht darin, Patienten, die sich außerhalb des spezialisierten Zentrums in einem Therapie-refraktären Schock oder unter Reanimation befinden, durch ein mobiles ECLS-System Hilfe zukommen zu lassen. Durch eine sofortige Kreislauf-Stabilisierung gewinnt das Ärzteteam Zeit („bridge-to-decision“), um weitere Therapieschritte durchzuführen – Intervention, Operation, Kunstherz und weitere Maßnahmen.

Sorge für die Patienten: Auch nach der Herzoperation

Prof. Albert weiß: Nur wenn eine Therapie schonend und gleichzeitig effektiv auf die individuellen Belange des Patienten zugeschnitten ist, kann sie wirklich erfolgreich sein. Durch seine Erfahrungen an Universitäten und Privatkliniken ist der empathische Facharzt natürlich auch bestens vertraut damit, selbst die komplexesten Sachverhalte so zu erklären, dass seine Patienten immer wissen, was mit ihnen geschieht. Besonders wichtig ist ihm aber auch, die Patienten nach der Operation im Verlauf wiederzusehen. Er weiß aus seiner langjährigen klinischen Tätigkeit ebenso wie aus wissenschaftlicher Beschäftigung mit dem Thema Lebensqualität nach herzchirurgischen Eingriffen, dass Patienten häufig eine besondere Beziehung zu ihren Operateur haben: Selbst Wochen nach der Operation kommen oft noch viele Fragen. So hat Prof. Albert es in Dortmund eingeführt, dass jeder Patient etwa zwei Monate nach der Operation noch einmal einen Termin zur Nachbesprechung der Operation bei seinem Operateur erhält. Dieses innovative Vorgehen unterstreicht, wie wichtig Prof. Albert das Wohl seiner Patienten ist.

Herzpatienten im Ruhrgebiet haben also nicht nur in Notfällen eine herausragende Anlaufstation.
